List at least 3 previous jobs beginning with your most recent employment experience. If you haven’t had 3 jobs, include odd jobs or volunteer experience so that you have at least 3 references with current contact information. Teachers or other adult non-family members can serve as references if you don’t have any of prior work experience.

I give my permission to do a criminal background check and to contact any or all of my previous schools and/or employers for full information. All of the foregoing information I have supplied in this application is a full and complete statement of the facts, and it is understood that if any falsification be discovered, it will constitute grounds for dismissal upon discovery thereof.
